– state which facility the person will be taken to 

– state the day and time the treatment facility informed you that a bed would be available for admission 

According to the paperwork, you must: – Provide a comprehensive description of the individual – Provide information on how to locate them – Specify which treatment facility the person would be transferred to and what day and time the treatment center informed you that a bed will be available for admittance
